Case Scenario:
A 48-year-old married male with two children presents to your clinic with complaints of unsatisfactory sexual performance and erectile dysfunction over the past year. He reports borderline low serum testosterone (Total testosterone: ~305 ng/dL) detected at a local clinic six months ago. He has tried over-the-counter Maca (Peruvian ginseng) supplements without significant improvement. He has no history of smoking, alcohol use, or major chronic diseases, but leads a sedentary lifestyle with a busy work schedule and is overweight.

Additional symptoms include mildly decreased libido and occasional low mood.

His physical and laboratory evaluations reveal:

• IIEF-5: 16 (mild ED)
• Erection hardness score (EHS): 2–3
• Blood pressure: 135/90 mmHg
• Waist circumference: 93 cm, Body mass index: 27.2 (kg/m²)
• Fasting glucose: 123 mg/dL, HbA1c: 6.9%
• Total testosterone: 298 ng/dL
• TG: 178 mg/dL, Total cholesterol: 225 mg/dL, HDL: 38 mg/dL
