Urologist Certificate (hereinafter
referred to as the Certificate) is valid
for six years and can be renewed for
another six years on expiry each time.

Applicant for renewal of certificate
should have participated in academic
activities or continuing education
(refer to rules governing points earned
through continuing education) and
accumulated at least 300 points during
the 6-year period of validity of the
certificate.

Applicant for renewal of certificate should submit the following:

Documents of proof meeting
conditions for renewal of
certificate as indicated in
Point 10. |

3. |
Three 2-inch portrait photos
without hat taken in the last
three months.

The Dept may charge for the expenses
incurred to conduct Screening and
renewal of Certificates based on the
actual spending. The above rule applies
to preliminary screening conducted by
the commissioned medical associations.
The medical associations are to report
the charge collected to the Dept.

The Dept shall notify all parties
involved of the results of the Screening
or renewal of certificates. Those who
pass the Screening or are granted
renewal of certificates may apply to the
Dept for issuance of certificates or
renewal of certificates according to
physicians classification and screening
rules.

Preliminary screening or checking of
renewal of certificates conducted by
medical associations is to be reviewed
or approved by the Dept. The medical
association shall notify the applicant
of the result. Applications for
certificates or renewal of certificates
shall be delivered to the Dept through
medical associations. |
